I haven't even attacked any civilians (yet), although I have noticed there is no takedown button for civilians, I presume I have to 'turn safety off' or something.

No, just go up to them and press Q. The prompt doesn't appear because they're not enemies, but you can still activate the takedown.

I like the fact that while it's clearly been established that punching people with your giant doomfists simply knocks them out (unlike mr sneak-lethal fucking tranq rifle *shakes fist*), the standard police response to you punching a civilian is "IMMEDIATELY KILL ON SIGHT".

Sorry, yes: I thought everyone had noticed this (I've been deliberately avoiding spoiler threads coz I'm still in detroit, exploiting the fuck out of the stupid weapon-selling system).
For absolutely no good reason, sometimes people you tranq will just..die. Not from falling off ledges or falling into fire or anything (though that works too), they just..pass out and then die in their sleep after an indeterminate time. You get credit for knocking them out, but it's still kinda annoying.
As Jonas points out, this is actually surprisingly realistic given that tranquilisers really really don't work like they do in games: "one dart size KOs all!", and are more of the "not quite enough drug: no effect, a little too much: dead" variety, and this isn't necessarily related to body-weight either: some people are just naturally more drug-resistant, some are naturally more sensitive (the difficulties in being able to judge and assess all this is why anaesthetists are highly-trained medical professionals).
BUT. If you're going to have something like that, then it should fucking state this at some point, ever. Which it doesn't. Thus I suspect it's a bug. Apparently the tazer can do this too sometimes (maybe you need a 'pacemaker detecting aug' to work out who is tazer-sensitive?).
