It looks very familiar, and I might have played it. But I can't remember the title. I do remember that the side-scrolling shooter genre was very popular in the days.

The five days are up - game was Hi-Octane, a DOS game that I just got running again on DOSBox. Haven't played it in a good eight years, at least.
